This rebate offer applies only to qualifying purchases of the products listed above when products are purchased in Canada between February 1, 2009 and September 30, 2009. Any applicable Canadian sales taxes (i.e. Goods and services Tax, Quebec Sales Tax and Harmonized Sales Tax) are included in the rebated face value.

Fill out this form completely, and submit it along with a copy of a valid store receipt with the qualifying products circled. Claims must be postmarked on or before October 15, 2009, and received by October 31, 2009. Offer applies to end user, original customer only. No resellers, distributors or dealers. Offer is not transferable. This offer is limited to one rebate per household, individual or organization. This rebate offer cannot be combined with other coupon or rebate offers. This rebate is in Canadian currency and is redeemable only in Canada. Offer open to residents of Canada only. Subject to all Federal, Provincial and local laws. No substitutions or extensions. Offer is subject to product availability. Sure-Gro Inc. and its agents have the right to substantiate submissions and to reject claims that do not comply with these terms. Handwritten receipts will not be accepted.

Sure-Gro Inc. is not responsible for lost, stolen, invalid or incomplete submissions. Claims will not be accepted if receipt shows retailer deducted the rebate amount at the time of purchase. Some retailers may require use of their own in-store coupon to claim this rebate; please check your receipt or ask the retailer at time of purchase. You should receive your rebate within 6 to 8 weeks from receipt of a properly completed claim. Keep copies of all materials submitted; originals become the property of Sure-Gro Inc.
